SOMERVILLE, NJ – The Somerset County Office on Aging & Disability Services is conducting a needs-assessment survey for Somerset County residents ages 60 years and above, people living with disabilities and caregivers. The objective is to gather information about how to improve community-based services and programs. The survey is available in print and online. The deadline for responses is Aug. 31.
“Everyone’s input is valuable and makes a difference in funding and program decisions made by the county’s Office on Aging and Disability Services,” said Somerset County Commissioner Paul M. Drake, liaison to the Department of Human Services. “Examples of currently funded programs include paratransit services, home health services, Project Lifesaver, nutrition programs, and the home-delivered meals program. All of these are extremely important to our communities.”
Needs Assessment Survey Information
All information given by the survey participant will remain confidential. If there is more than one eligible member in the household, each person should complete their survey. Caregivers can complete the survey for individuals who are unable to complete the form.

Older Americans Act of 1965
In compliance with the Older Americans Act of 1965, the Office on Aging and Disability conducts a survey annually to determine the needs of the aging community and people with disabilities. The Office on Aging and Disability includes those survey results in the Service Area Plan Contract, which is developed every three years and then submitted for approval to the State Department of Human Services Division of Aging Service. The Area Plan helps agencies identify needs and trends and implement programs and services to address those needs.

The Somerset County Office on Aging & Disability Services serves as an ADRC-designated entity that streamlines access to long-term care support services, health promotion and education. The agency is committed to promoting health, independence, and dignity and providing choices for adults ages 60 years plus, and adults ages 18 years plus with physical disabilities, veterans and caregivers in Somerset County by providing access to diverse services to meet individual needs in a fiscally responsible manner.
