2020 Child Health Conference Vaccines for Children Program Flyer and Questionnaire

Please find our flyer for the 2020 Child Health Conference Vaccines for Children program which will restart today, Thursday, July 9, 2020. Also attached is a sample of a questionnaire which will be used at the clinic to screen participants. Be assured that all infection control practices will be in effect.

The goal of this clinic is to provide immunizations for vaccine-preventable diseases free of charge for children from birth to age 18 who do not have health insurance. We know that due to the Covid-19 pandemic some children or adolescents have not received their immunizations needed. Kindly post and distribute the attached flyer to all in your community who may benefit from the program.

Parents and Guardians who need to access this service for a child will need to make an appointment by phoning the Community Visiting Nurse Association at: 908-725-9355, ext. 0.
